18 week support pilot underway

The Map of Medicine is starting a pilot project with South Bedfordshire Local Health Community (LHC) to model the use of a clinical knowledge tool to support the 18 week programme.

The Map of Medicine, with its support of both primary and secondary care patient pathways, is an ideal mechanism to allow clinicians and service redesign personnel to come together and discuss the best way to reconfigure their service to meet the 18 week referral to treatment target.

Pathways adapted for local needs

It is expected that the first localised clinical pathways will start to appear on the Map of Medicine in late January 2007.

A local view

Based in Luton and Dunstable NHS Trust, the project will focus on COPD and ENT. The clinical pathways will be reviewed by the Trust and PCT and localisation work will be undertaken. The revised pathways will then be published into a local view of the Map of Medicine to allow all clinicians in the LHC to become aware of the local pathways and processes.

A structured review methodology

The pathway review process is a structured method designed to provide a rapid review and consensus for changes to pathways. This process will be used to support and ensure appropriate governance of the activities undertaken for localisation. Conwy & Denbighshire NHS Trust in North Wales is also using this process to support service redesign work being undertaken to meet the ACCESS 2009 initiative in Wales.

Localisation has begun on ENT and Orthopedics. It is expected that the first localised clinical pathways will start to appear on the Map of Medicine in late January 2007.
